Health and Wellness

Student health and well-being is very important to the faculty at SHS, and students must have appropriate forms on file.

We host mental health professionals from both Community Health and Marion County Health Department.

We also host a Raphael Health Clinic on campus for students staffed with a full-time nurse who oversees all areas of our medical care for our students. The Raphael Health Center is located two blocks away on 34th street at Central Ave where full service health care is available including dental, medical, and mental health care.

Shortridge retains student health records and additional documentation. If your child requires medication—either self-administered or administered by a nurse—please complete and return the below forms.
