Currently, IPS is offering interscholastic sports programs at the middle school and high school levels. Elementary level programming is tentatively scheduled to return for the 2021-2022 school year.

Parents/Guardians of participants are required to review and sign this form in order for their student to participate in IPS Athletics programming in the 2020-2021 school year.

During this time, it is important for the families of all student athletes to be aware of the risks associated with participation and the possibility of contracting COVID-19. This document goes over that risk and provides some additional resources that we recommend families review. Participating students and their parents are required to review, sign, and submit this form.
Health Portal w/ HIPAA and IHSAA Health Update Questionnaire
This website provides a link to two required forms:
– The HIPAA form which permits our IU Health certified athletic trainers to properly serve student athletes.
– The IHSAA Health History Update Questionnaire which will serve in place of a Pre-Participation Physical Evaluation (PPE) for the 2020-2021 school year as long as the participating student has a physical from the 2019-2020 school year and has not had any of the red flag issues that require a new IHSAA PPE.
